Please submit your 8-digit Lancaster University Student ID, and your first and last name in full. 
You can access your LU email address and University ID information through Lancaster University's student portal (https://portal.lancaster.ac.uk). Where your name is visible on the top right of the page, press the drop-down arrow next to this and select 'my profile'.

Please note: the above list of assessments does not include formative assessments (those which don't count towards your final grade, such as Direct Observation 1 (part of FLSW914a.3 or 901.3) and Direct Observation 3 (part of FLSW914b.2 or 903.3)). If you cannot submit these by the deadline please discuss it with your Practice Tutor and email academic.support@thefrontline.org.uk

Please provide the full details of your exceptional circumstances, including key dates, and what the impact has been on your assignment. 

If requesting an extension, it is helpful if you can explain what progress you have made on your assignment to date, and what plans you have in place to complete it. Your Practice Tutor / Dissertation Supervisor will be asked to comment on your request.

If you have an ILSP and have ticked the relevant box above, you do not need to redisclose your disability/SpLD/health condition to us, as your ILSP will be available to the panel. Please do provide the other information requested above so that we can consider an appropriate adjustment.
