Information on why we collect this data and how it will be shared

The purpose of this feedback form is to share themes and insights from your coaching and flag any concerns for the team

Only a select few Frontline staff will be able to see your individual responses linked to your name. In order to help us illustrate and prove our impact we may share information with the Department for Education, funders, trusted third parties, on our website and in published or unpublished reports. In these cases ratings will be aggregated and any quotes used will be anonymous so you will not be individually identifiable.

4. Are there any issues that the coachee would like flagged and passed on to the Programme Team? E.g. supervision issues, high number of cases, Masters concerns. If you wish to feedback on a specific participants experience no contact will be made with the relevant party (e.g. local authority/practice tutor) before a member of the team has contacted the mentioned ASYE to discuss this further.
